Everything needed to complete your trip is inside the flat rate. The airport fee applies to arrivals (pickups), not departures (drop-offs) — and when it applies, it's already inside your quote. Toronto Pearson charges pre-arranged ground-transport operators a fee to enter and stage for meet-and-greet pickups. On an airport pickup, that fee plus the meet & greet inside arrivals is built into the flat rate you're quoted.
A departure — being driven from home or office to the terminal — has no airport pickup fee, which is why a drop-off quote is often slightly lower than the reverse pickup for the same route. Either way, the quote you see is complete.
